BraveWork Psychotherapy is committed to protecting the privacy and confidentiality of your Personal Health Information (PHI) in accordance with Ontario's Personal Health Information Protection Act (PHIPA, 2004).
1. Collection of Information
We collect information necessary for your care, including contact details, health history, and session notes.
2. Purpose of Use
Your PHI is used to provide clinical treatment, maintain records as required by the CRPO, and process billing.
3. Limits of Confidentiality
Information is kept strictly confidential except where disclosure is required by law:
- (a) Risk of imminent harm to self or others
- (b) Suspected abuse or neglect of a child
- (c) Sexual abuse by a regulated health professional
- (d) Subpoena by a court of law
4. Data Storage
We use JaneApp, a PHIPA-compliant platform, for all electronic records. Records are retained for 10 years following the last clinical contact (or 10 years past a minor's 18th birthday).
5. Your Rights
You have the right to access your records and request corrections.
